Main St., #348, Ann Arbor, MI  48103, or 3) scan the form and email it to wholesale@trilliumnutritionals.com.  Included on the form are details of our program including prices, minimum orders, and terms.  We will notify you as soon as you are approved as a Certified Dealer, assign your account number, and take your first order.  It’s that simple!
